UK guidelines cutaneous melanoma

Breslow, Clark and AJCC staging

Breslow's Depth

A staging system previously used to help prognostically score melanoma. The higher the stage the worse the prognosis

  • Stage I - depth <0.75mm

  • Stage II - depth 0.76-1.50mm

  • Stage 3 - depth 1.51-4.00mm

  • Stage IV - >4mm

​

Clark's Level

Often used in conjunction with Breslow' depth, describing invasion of the melanoma lesion. The higher the level, the poorer prognostic outlook

  • Level 1 - melanoma in situ/confined to epidermis

  • Level 2 - invasion into the papillary dermis

  • Level 3 - invasion to the junction of the papillary and reticular dermis

  • Level 4 - invasion into the reticular dermis

  • Level 5 - invasion into subcutaneous fat

Since 2010, melanoma has been staged via the AJCC cancer staging. This reflects the traditional TNM approach, with further information deemed whether there is ulceration, microsatellite metasteses and LDH levels

​
