Remember the importance of breaking the GCS into E, V and M. Please also score based on the patient's best response in each category. Minimum score = 3 (totally unresponsive)

Maximum score = 15

E - Eye opening response

​

  1. No response

  2. To pain

  3. To voice

  4. Spontaneously

​

V - Best verbal response

  1. No response

  2. Incomprehensible sounds

  3. Inappropriate words

  4. Confused

  5. Orientated to time, place and person

​

M - Best motor response

  1. No response

  2. Abnormal extension (decerebrate)

  3. Abnormal flexion (decorticate)

  4. Flexion withdrawal from pain

  5. Moves to localised pain

  6. Obeys commands
